Soluna's Affiliate Network connects your organization with California youth ages 13 to 25 who need exactly what you offer — matched to your capacity, at no cost to join.
Support that goes beyond Soluna's service
When a young person needs more than coaching or self-guided tools can offer — therapy, medical care, housing, food — Soluna's Care Navigation team steps in. Our trained Care Coordinators stay with the young person through the whole process: checking insurance, completing forms, booking appointments, and following up afterward to help ensure they successfully connect with the services they need.
As an Affiliate Network partner, your organization becomes one of the trusted local resources our Care Navigators turn to for that support. We take the time to understand what you offer, so every referral arrives warm and closed-loop — no young person handed off and forgotten, and no thread left open on either end.
Built on partnership, not competition
We don't replace your services or take business from you — we partner with you. Our goal is to streamline access to the work you're already doing, so no young person slips through the cracks along the way.
Is your organization a fit?
- You might be a great match for the Affiliate Network if you: direct services, resources, or support to California youth ages 13-25
- Offer mental, physical, or community-based services — including medical or behavioral healthcare or SDOH supports like food, housing, or transportation
- Have program capacity to support new clients
- Are willing to provide program information on Soluna’s FindHelp repository (soluna.findhelp.com)
- Bilingual or multilingual services
- Culturally responsive care, including support for BIPOC and LGBTQ+ communities

Tell us about your organization. We’ll follow up within 3 business days to schedule a conversation and see if we’re a fit.
Configure
If it’s a match, we’ll finalize a partnership agreement and get your organization set up to receive referrals through the Soluna FindHelp platform..
Collaborate
Join a short orientation with our Care Coordinators to walk through your services and align on referral logistics — then we keep working together to improve the process over time.
