Support That Goes Beyond Soluna's Service

When additional help is needed, we guide users to trusted community organizations and service agencies across California.

Whether it's therapy, traditional medical needs, or access to community resources like housing and food, our Care Navigation team works directly with young people to match them with the right service. They support them through every step of the process, including contacting insurance and reviewing coverage, completing forms, booking appointments, and checking in afterward to make sure the need is met.

As an Affiliate Network partner, you become one of the trusted local resources our Care Navigation team turns to when youth need additional support. We take the time to understand your services so referrals are smooth, accurate, and timely.

We don’t replace your services or take business from you — we partner with you. Our goal is to streamline access to the vital support you already provide!

Your organization might be a fit for our Affiliate Network if:

You provide direct services, resources, or support to California youth ages 13-25
You provide youth with mental, physical, or community-based services — including medical or behavioral healthcare or SDOH supports like food, housing, or transportation
You have program capacity to support new clients
You are willing to provide program information on Soluna.findhelp.com
Ideally, you will also offer bilingual or multilingual services, in addition to culturally sensitive services, including support for marginalized communities such as BIPOC and LGBTQ+ populations

If we’re a match, we’ll move forward with a partnership agreement. We’ll also guide you through setting up your organization to accept referrals on the Soluna Findhelp platform.

Collaborate

Join a brief orientation with our Care Coordinators to share your services and align on referral logistics. From there, we’ll keep collaborating to improve our process and better support youth together.
